Description
Using extensive examples, exercises, and real life scenarios, this course teaches how to document functional outcomes in a clear and logical progression. Documentation in this course is based on the recent APTA endorsement of the International Classification for Functioning, Disability, and Health Model (ICF). Essentials of documentation, clinical decision making and the initial evaluation format are covered in detail. The therapist will learn how to document reason for referral, health conditions, participation restrictions, activity limitations, and impairments. Not only are necessary components included for documenting initial evaluations, progress notes, discharge summaries, goals and plan of care, but special sections are included for documentation in pediatric settings, Medicare settings, and how to write letters to third party payers to justify equipment or services. Though this course can be beneficial to both the PT and PTA, the major focus of this course is on the initial evaluation format since it the most critical in establishing a framework for clinical decision making. This is a practical guide that will appeal to the practicing therapist who desires to improve reimbursement, communication, and protect against litigation.
